Thank you for your interest in planning your next vacation with Cruise Planners -Travel Trends by Sue. We put a tremendous amount of time and effort into curating and designing the trip for you and those traveling with you.
To do this effectively, we need YOUR help! This questionnaire is designed to help you be as specific as possible with your information about the type of travel you're interested in, when you want to travel, and where you want to travel. The more specific you are about your travel wants, the better we can plan for your trip. If we have already had our trip planning consultation, feel free to add any additional information that you may not have already shared with me.

Name Field - please provide your name as it appears on your government issued travel document - such as a passport, driver's license, or birth certificate.
Comments Field - Please provide as much detail about the type of trip you are seeking - including destination, any special celebrations, or anything else that can assist with the planning of this trip.
Additional Family Members - be sure to provide names and birthdates as they appear on passports for family members traveling with you along with any mileage program membership numbers in the Comment Box.
For quality control purposes, please email me copies of your passports as names and birthdates must be accurate.
